Passage Dual is an (HMO SNP) plan with a separate Passage network. With this plan, you’re required to get your care from doctors and hospitals in the Passage network, except for emergency and urgent care situations. Also, to join Passage Dual, your primary care provider (PCP) must be in the Passage network and accept Medicaid.
